Job summary

Responsibilities

• Ensure safe, effective discharge planning and participate in interdisciplinary rounds.
• Comply with CMS mandated forms and compliance expectations.

IMC is currently hiringa Case Manager, on a full time basis. The Case Manager is responsible for the coordination of services through the continuum of care that balances the delivery of quality patient care with the cost-effective utilization of resources. Supports collaboration and communication of patient/family and entire healthcare team regarding the patient’s plan of care and continued needs post-discharge and facilitates those services required.

EDUCATION

• Graduate of state approved School of Nursing - Or -
• Graduate of a state approved School of Social Work

LICENSURE/CERTIFICATIONS

• Current license as an RN in the state of Louisiana
• Current license as a LMSW in the state of Louisiana

TRAINING/EXPERIENCE

• Previous experience in Case Management preferred

RESPONSIBILITIES

• Conduct on-going clinical reviews toensure appropriate level of care and necessity of continued stay.
• Reports clinical reviews to third partypayer sources as required/requested.

• Ensures that the most effective andlevel appropriate discharge plan is in place for all assigned patients to promotesafety in the post-acute setting.
• Actively involved in interdisciplinaryrounding meetings.
• Complies with all complianceexpectations.
• Knowledgeable of all CMS Mandated Forms.
