Job summary

Iberia Medical Center in New Iberia, LA seeks a Registered Nurse - Care Manager to apply nursing process and coordinate care for patients under supervision of the Unit Director and CNO. You will interact with patients, families, and the health team to ensure appropriate care across the continuum.

Requirements include graduation from an approved School of Nursing and current Louisiana RN license. Prior experience is preferred but not required.

Qualifications

• Graduate of state approved School of Nursing.
• Current RN license to practice in Louisiana.
• Previous nursing experience preferred but not required.

Responsibilities

• Be available by telephone during working hours.
• Act as liaison between patients, physicians, and referral sources.
• Be knowledgeable of all assigned patients.
• Conduct ongoing clinical reviews to ensure appropriate level of care and necessity of continued stay.
• Report clinical reviews to third-party payer sources as required or requested.
• Post authorizations into the computer as needed.
• Make appropriate referrals as ordered by the MD.

IMC is currently hiring a Registered Nurse - Care Manager, who under the general supervision of the Unit Director and with the authority of the Chief Nursing Officer, will apply nursing process (assessment, planning, intervention and evaluation) in caring for patients. Effectively interact with patient, significant others, and other heath team members. Demonstrates knowledge and skills necessary to provide care based on physical, psycho/social, education, safety and related criteria for the newborn, pediatric, adolescent, adult and geriatric population.

EDUCATION

• Graduate of state approved School of Nursing.

LICENSURE/CERTIFICATIONS

• Current licenses as a RN in the state of Louisiana.

Training/Experience

• Previous experience preferred, but not required.
