Job summary

Qualifications

Responsibilities

• Be available by telephone during working hours.
• Act as liaison between patients, physicians, and referral sources.
• Know all assigned patients and conduct ongoing clinical reviews for appropriate level of care.
• Post authorizations into computer and make referrals as ordered by MD.
• Ensure safe, effective discharge planning and participate in interdisciplinary rounds.
• Comply with CMS mandated forms and compliance expectations.
