Job summary

Iberia Medical Center in New Iberia, LA is hiring a Registered Nurse - Care Manager to apply nursing process and coordinate care for patients. Role involves liaising with patients, families, physicians and referral sources, and ensuring appropriate discharge planning.

Under supervision of Unit Director and CNO, duties include clinical reviews, referrals, and compliance with CMS forms, serving diverse patient populations.

Qualifications

• Graduate of state approved School of Nursing.
• Current RN license in Louisiana.
• Previous experience preferred, but not required.

Responsibilities

• Be available by telephone during working hours
• Act as liason between patients, physicians, and referral sources.
• Be knowledgeable of all assigned patients.
• Conduct on-going clinical reviews to ensure appropriate level of care and necessity of continued stay.
• Reports clinical reviews to third party payer sources as required/requested.
• Post authorizations into computer as needed.
• Make appropriate referrals as ordered per MD.
• Ensures that the most effective and level appropriate discharge plan is in place for all assigned patients to promote safety in the post-acute setting.
• Actively involved in interdisciplinary rounding meetings.
• Complies with all compliance expectations.
• Knowledgeable of all CMS Mandated Forms.

IMC is currently hiring a Registered Nurse - Care Manager, who under the general supervision of the Unit Director and with the authority of the Chief Nursing Officer, will apply nursing process (assessment, planning, intervention and evaluation) in caring for patients. Effectively interact with patient, significant others, and other heath team members. Demonstrates knowledge and skills necessary to provide care based on physical, psycho/social, education, safety and related criteria for the newborn, pediatric, adolescent, adult and geriatric population.

EDUCATION

• Graduate of state approved School of Nursing.

LICENSURE/CERTIFICATIONS

• Current licenses as a RN in the state of Louisiana.
