Job summary

CrossNet Ministries in New Holland, PA, is seeking a Case Manager to walk alongside individuals and families, providing trauma-aware, strengths-based support and connecting them with CrossNet programs and community resources.

You will conduct intakes, develop individualized service plans, document progress, and collaborate with program staff to improve participant outcomes while maintaining confidentiality and accurate records in a Christ-centered nonprofit setting.

Qualifications

• Bring 2+ years of experience in case management, social services, or a community support role.
• Holds a bachelor's degree; Social Work, Human Services, Psychology, or a related field is preferred.
• Demonstrates faith-driven ethics and ability to work in a Christ-centered setting.

Responsibilities

• Provide trauma-aware, strengths-based case management to individuals and families, managing multiple cases.
• Conduct intakes, assessments, and ongoing check-ins to set goals and track progress.
• Develop individualized service plans and connect participants with CrossNet programs and community resources.
• Document interactions, referrals, services, and outcomes while maintaining confidentiality.
• Collaborate with staff and leadership to coordinate care and support reporting needs.
• Improve workflows, intake processes, and documentation by identifying trends and gaps.
• Support programs and community initiatives including outreach, food distribution, and emergency assistance.
• Represent CrossNet during home visits, community meetings, and partner collaborations.

What You'll Do as a Case Manager

• Provide trauma-aware, strengths-based case management to individuals and families, managing an average of 15 cases each month.
• Conduct participant intakes, assessments, and ongoing check-ins to identify needs, establish goals, and track progress.
• Develop individualized service plans and connect participants with CrossNet programs, community resources, and external agencies.
• Document participant interactions, referrals, services, and outcomes while maintaining confidentiality and accurate records.
• Collaborate with other Case Managers, program staff, and leadership to coordinate participant care, gather data, and support reporting needs.
• Improve Participant Services workflows, intake processes, and documentation systems by helping identify trends, gaps, and opportunities.
• Support CrossNet programs and community initiatives, including food distribution, emergency assistance, outreach, and seasonal programs.
• Represent CrossNet Ministries during local home visits, community meetings, and partner collaborations throughout Eastern Lancaster County.

• Experienced: Bring 2+ years of experience in case management, social services, or a community support role.
• Educated: Hold a bachelor's degree; Social Work, Human Services, Psychology, or a related field is preferred.
• Compassionate & Relational: Approach individuals and families with patience and empathy, building trust while navigating complex situations professionally.
• Collaborative & Adaptable: Work effectively as part of a team while managing independent responsibilities and adapting to evolving program and participant needs.
• Organized & Dependable: Maintain accurate records, manage participant data, protect confidentiality, and demonstrate strong attention to detail and follow-through.
