  • Collaborate with healthcare providers, social service agencies, and child welfare organizations to ensure integrated care delivery.
  • Develop and implement discharge plans that promote client safety, independence, and continuity of care post-service.
  • Maintain accurate documentation of client interactions, care plans, and progress notes in compliance with standards.
  • Advocate for clients to access necessary services while respecting rights and preferences.
  • Monitor and evaluate client outcomes to adjust interventions as needed.

About the Role:

The Case Manager plays a pivotal role in coordinating comprehensive care and support services for individuals and families within the health care and social assistance sector. This position is responsible for assessing client needs, developing personalized care plans, and facilitating access to appropriate resources to promote well-being and recovery. The Case Manager collaborates closely with multidisciplinary teams, including mental health professionals, social workers, and community organizations, to ensure seamless service delivery and continuity of care. A key outcome of this role is to empower clients through advocacy, crisis intervention, and conflict resolution, ultimately improving their quality of life and supporting successful discharge planning. The Case Manager must maintain detailed documentation and monitor client progress to adapt care strategies effectively and meet evolving needs.

Minimum Qualifications:

  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
  • Minimum of two years of experience in case management or social services within a healthcare or community setting.

  • Strong understanding of discharge planning processes and community resource networks.
