Job summary

Housing Families Inc. is seeking a Case Manager to work directly with individuals experiencing homelessness, providing ongoing support, housing search assistance, and access to community resources.

You will conduct needs assessments, develop rehousing strategies, and coordinate with service providers. The role requires strong communication, empathy, and a trauma-informed approach, with the ability to work 40 hours weekly and pass CORI/SORI background checks.

Responsibilities

  • Meet regularly with shelter guests and conduct thorough needs assessments.
  • Develop attainable rehousing strategies and assist in mitigating barriers.
  • Provide support and referrals and maintain contact with other service providers.
  • Conduct routine room inspections to ensure properties are maintained.
  • Complete documentation to comply with program requirements and state regulations.
  • Manage crises as they arise and participate in staff trainings.

Job description

Role Purpose: The Case Manager will work directly with individuals experiencing homelessness to provide ongoing support, housing search and access to community resources.Responsibilities:Ensure all individuals are served and treated with dignity and respect.Ensure all services are provided in compliance with various funding sources.Ensure accurate records are maintained for all clients.Develop relationships with individuals who are unhoused or recently housed and facing issues such as mental health concerns, substance use, physical disability, history of trauma and/or domestic violence, etc.Tasks will include but are not limited to meeting regularly with shelter guests and completing thorough needs assessments, developing an attainable rehousing strategy, aiding with mitigating rehousing barriers, providing support and referrals, and ensuring regular contact with other service providers.Conduct routine room inspections to ensure the property is maintained.Complete all necessary paperwork and documentation needed to be in compliance with program requirements and state regulations.Manage crises as they arise, and other duties as assigned.Participate in All Staff meetings and other identified trainings and meetings.Experience working directly with clients in the human services field.The ability to hear and incorporate feedback quickly, empathically de-escalate emotional reactions and think critically.Strong assessment, problem solving and negotiating skillsThe ability to remain calm and responsive during crisis.A positive attitude with the ability to be flexible and adaptable to changes.Attention to detail.Compelling advocacy skills.Ability to work with a diverse team and population.Working knowledge of the impact of trauma, homelessness, and Motivational Interviewing.Working knowledge of various resources and public benefitsFluent in EnglishBilingual Spanish or Haitian Creole a plus.Eligibility Requirements:Ability to be at work on time daily.Attendance at meetings, intakes etc , the employee is expected to be on time on all occasions.Must be willing to work 40 hours per week, and adapt schedule to meet the needs of individualsMust have unrestricted work authorization to work in the United StatesMust be 18 years or olderAbility to form strong interpersonal relationships with people from diverse backgrounds (staff, clients, etc.)Must be able to pass a Criminal Offender Record Information (CORI) and Sex Offender Registry Information (SORI) background checkPhysical Requirements:Must be able to sit for prolonged periods of time at a desk while working on a computer.It is unlawful in Massachusetts to require or administer a lie detector test as a condition of employment or continued employment.
