Job summary

Verity Behavioral Services & Primary Care is seeking a compassionate Case Manager to support children, adolescents, and adults in behavioral health and supportive services. The role involves assessing needs, coordinating resources, and advocating for continuous care to improve outcomes.

You will work closely with clinicians, families, schools, medical providers, and community partners to address psychosocial, educational, and functional needs, while maintaining compliant documentation in the EHR.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, Sociology, or related field is required.
• Case Management experience in behavioral health or social services preferred.
• Knowledge of community resources and social service systems.
• Strong organizational, communication, and documentation skills.
• Ability to work independently and as part of a multidisciplinary team.
• Experience with children, adolescents, and/or families preferred.
• Valid driver’s license and reliable transportation if applicable.

Responsibilities

• Conduct comprehensive psychosocial and needs assessments
• Develop, implement, and monitor individualized service plans
• Coordinate care with therapists, psychiatrists, primary care providers, schools, and community agencies
• Link clients to community resources (housing, education, medical care, food assistance, employment)
• Advocate for clients and families to ensure access to appropriate services
• Monitor progress toward treatment goals and adjust plans as needed
• Maintain accurate, timely, and compliant documentation in the EHR
• Participate in treatment team meetings and case conferences
• Provide crisis intervention support and referrals when necessary
• Ensure compliance with Ohio Medicaid, agency policies, and ethical standards

Verity Behavioral Services & Primary Care is seeking a compassionate, organized, and client-focused Case Manager to support children, adolescents, and adults receiving behavioral health and supportive services. The Case Manager will assess client needs, coordinate community resources, and advocate for individuals and families to ensure continuity of care and improved outcomes.

This role works closely with clinicians, families, schools, medical providers, and community partners to address psychosocial, educational, and functional needs.

Key Responsibilities
