The Adult Case Manager is responsible for delivering Community Psychiatric Support Treatment
(CPST) services to adults age 18 and older with complex and multiple service needs. This role includes
developing collaborative, goal-oriented treatment plans and supporting clients in achieving
identified outcomes. The position requires effective time management, the ability to work
independently in community-based settings, and the capacity to collaborate within a
multidisciplinary team to ensure coordinated, client-centered care.
The Adult Case Manager works under the direct reporting of the Assistant Managers of Traditional
Case Management and in guidance / collaboration with other PHS Leaders.

Experience providing case management to adults with severe and persistent mental illness and dual diagnosed populations. A Bachelor’s degree in counseling, social work, human services or a related field or an Associate’s degree in human services or a related field with at least 3 years of experience working in the field.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. The ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
To perform this job successfully, an individual should have the ability to gain knowledge of current
practice management system, electronic medical record, Microsoft Word, text paging, Internet.
Clinical licensure, Ohio nursing license.
Excellent communications skills, both written and verbal. Ability to work effectively and independently with all levels of clinical and administrative staff within the health centers and with community leaders.
Strong leadership qualities and leadership experience. Demonstrates program development and implementation skills. Ability to represent the organization effectively in a variety
of settings and with diverse communities. Demonstrates understanding and appreciation for diverse cultures.
