Job summary

Community Support Services is hiring a Case Manager in Akron, OH. This full-time role focuses on enhancing clinical services by offering practical help, teaching recovery-based skills, and coordinating ISPs to meet client goals.

The position requires LSW/LPC licensure, a valid Ohio driver’s license, and strong organizational and communication abilities, with opportunities to work within a collaborative team dedicated to recovery-oriented care.

Qualifications

• Minimum of an LSW or LPC licensure.
• Prior experience in case management with individuals with severe mental illness preferred.
• A car and a valid Ohio driver’s license and insurability through the Agency’s insurance carrier required.

Responsibilities

• Serve on a team of Case Managers, Recovery Specialists, and medical staff; attend team meetings.
• Monitor and coordinate evaluations and assessments to identify client needs.
• Assist clients to identify and minimize psychiatric symptoms and enable self-management.
• Develop an Interdisciplinary Service Plan (ISP) with necessary services to reach objectives.
• Connect clients with community resources to build coping skills.
• Help clients grow social support networks of relatives, friends, and volunteers.
• Accompany clients to appointments and assist with daily living skills.
• Maintain relevant documentation.

Position Type: Full Time (40 Hours)

Salary Range: $24.00 - $31.75 Hourly

The Opportunity: We are seeking to hire a Case Manager to join one of our regional treatment teams. This individual enhances the provision of clinical services to clients by offering practical help, building community support, teaching recovery-based skills, and monitoring services.

What You'll Do:

• Serve on a team built up of fellow Case Managers, Recovery Specialists, and medical staff to navigate the needs and care of your clients, attending team meetings on a regular basis.
• Monitor and coordinate the delivery of necessary evaluations and assessments in order to identify each person's needs.
• Assist individuals to identify and minimize the effects of their psychiatric symptoms, enabling individuals to self-manage their illness.
• Coordinate and participate in the development of an Interdisciplinary Service Plan (ISP) that shall include services and assistance necessary to achieve specific objectives.
• Connect clients with community resources to help them develop interpersonal and community coping skills.
• Assist the individual in fostering and increasing their social support networks of relatives, friends, and/or volunteers.
• Accompany individuals to appointments and activities, assisting in daily living skills training as needed.
• Maintain relevant documentation.

This position is full-time (40 hours a week) working normal business hours.
