Job Summary
Case Manager CCBHC. The role completes all required educational interventions, assists in the completion of screening processes, accurately reports information to team members and clients/families, and participates in the development and implementation of service plans and crisis interventions.

Training Requirements
Annual BLS required. Existing incumbents with Heart Saver First Aid CPR/AED must renew BLS. All new hires must have BLS within 90 days of hire or transfer. CPI also required within 90 days of hire or transfer.
Duties and Responsibilities
- Complete all required educational interventions.
- Assist in the completion of screening forms.
- Accurately report information to team members and participate with them and clients/families in the completion of a Service Plan within established time frames.
- Demonstrate knowledge of crisis intervention and behavior management of aggressive behavior.
- Assist in the implementation of psycho‑educational programs.
- Accurately document all client interactions within established time frames.
- Provide services to clients/families in their native language or procure the services of a translator.
- Seek supervision in a professional manner and document supervision.
- Participate with team members and clients/families in discharge planning.
- Develop an adequate professional relationship with all referral sources and relevant community agencies.
- Respond adequately to the needs of clients, families, and referral sources.
- Provide core services as assigned.
- Assist in data collection for each assigned client, including:
- Relationship of the client’s physical status to dependence.
- Nature of comparison to use alcohol and/or other drugs.
- Intensity of the client’s mental preoccupation with using alcohol and/or drugs.
- Establish and maintain positive community supports.
- Maintain compliance with DMHAS licensing regulations, including confidentiality of drug and alcohol clients.
- Participate in treatment team conferences to support best practices.
- Model and support clients and staff in health and wellness activities and healthy coping skills.
Requirements
- Education: Bachelor’s degree in a behavioral health field required.
- Experience: At least one year in a behavioral health field or an appropriate internship required.
- Understanding and support of a trauma‑informed system of care.
- Preferred: Experience with illness management and recovery (IMR) and motivational interviewing (MI).
- Excellent computer skills required.
- Strong verbal and written communication skills required.
- Self‑directed and flexible to meet the needs of administration and management.
- Sensitivity to a culturally diverse client population and workplace.

Licensure / Certification
- Current and valid NJ driver’s license.
