Job summary

CPC Integrated Health is seeking a Care Manager to work across clinic-based environments, community settings, and clients' homes to coordinate services and support long-term recovery. The role involves field-based outreach, crisis intervention, psychoeducation, and skill-building to help individuals access essential resources and achieve wellness.

Qualifications

  • Bachelor's degree in social work, psychology, human services, or related field required.
  • Minimum of one year of experience in case management, behavioral health, or social services.
  • Two or more years of direct experience with SUD or co-occurring conditions in home-based or community outreach preferred.
  • Master's degree preferred and CADC preferred.

Responsibilities

  • Manage caseload, meet productivity standards, conduct home visits and community-based outreach.
  • Collaborate with clients to create and carry out individualized service plans and navigate healthcare, housing, legal, and employment systems.
  • Coordinate transportation and accompany clients to key appointments to foster empowerment and follow-through.
  • Deliver psychoeducation to clients and families, coach daily living skills, and respond to crisis with de-escalation/intervention techniques.
  • Advocate for clients by linking with peer supports, community resources, and specialized care providers.

Job description

The Care Manager is an integral member of the interdisciplinary team involved with each individual's care at CPC. The Care Manager works directly with clients in clinic-based environments, community settings, and clients' homes to coordinate services, establish goals, and support individuals in achieving wellness, recovery, and productive lives. This position involves field-based outreach to ensure access to essential services and reduce barriers to care.

The Care Manager actively delivers case management, crisis intervention, psychoeducation, and skill-building support, directly connecting individuals to vital community resources. They work collaboratively with clients, their families, support systems, clinical teams, and community providers to achieve positive outcomes and promote long-term recovery.

LICENSE/CERTIFICATIONS:
  • Valid NJ Driver's License
MINIMAL QUALIFICATIONS:
  • Education: Bachelor's degree in social work, psychology, human services, or a related field required.
  • Experience: Minimum of one (1) year of experience in case management, behavioral health, or social services.
PREFERRED QUALIFICATIONS:
  • Two or more years of direct experience working with people experiencing Substance Use Disorder (SUD) or significant co-occurring mental health and substance use in a home-based or community outreach setting preferred.
  • Master's degree preferred.
  • CADC preferred.
MAJOR RESPONSIBILITIES:
  • Case Management & Coordination:
  • Manage a caseload, meet productivity standards, and complete required documentation. Conduct home visits and community-based outreach.
  • Collaborate with clients to create and carry out individualized service plans. Guide and support clients as they navigate healthcare, housing, legal, and employment systems.
  • Coordinate transportation and, when appropriate, accompany clients to key appointments to foster empowerment and follow-through. Provide assistance with walk-in intake process
2. Direct Client Support & Skill Development:
  • Deliver psychoeducation to clients and families on mental health, substance use, and healthy coping strategies. Coach clients in building daily living skills.
  • Respond to crisis within the clinic setting, using de-escalation and intervention strategies, helping clients regain a sense of safety and control. Collaborate on safety plans, relapse prevention tools, and wellness strategies that promote long-term recovery.
  • Partner with clients on housing-related needs.
3. Advocacy & Linkage to Community Resources:
  • Connect clients with peer support, community programs, and natural support systems. Accompany and assist clients in gaining access to benefits, social services, vocational support, and legal resources.
  • Directly connect clients to specialized care providers, medical professionals, and crisis services that align with each client's needs and goals, particularly those with co-occurring SMI and SUD or chronic physical health conditions.
  • Actively support client care by utilizing the Health Information Exchange (HIE) system to guide appropriate follow-up, managing the medication line, assisting with collecting client vitals and other CCBHC measures.
4. Documentation & Compliance:
  • Maintain clear, timely, and accurate records in the electronic health record (EHR) system, in accordance with regulatory standards.
  • Monitor and document client progress. Uphold compliance with all federal, state, and local regulations, including those related to Medicaid and grant-funded services.
  • Ensure strict adherence to HIPAA regulations and organizational privacy policies. Assist with completing and managing authorization process and all other components of the NJSAMS system.
ESSENTIAL COMPETENCIES:
  • Ability to work in a community-based clinic setting, as well as homes, shelters, and other non-clinical environments.
  • Strong understanding of mental health, substance use, and social determinants of health.
  • Excellent problem-solving and organizational skills.
  • Ability to engage and build trusting relationships with diverse populations.
  • Proficiency in electronic health records (EHR), Microsoft Office, and online resource navigation.
  • Commitment to trauma-informed, person-centered, and culturally competent care.
  • Flexibility in scheduling to meet client and program needs.
  • An ability to independently travel to a variety of locations using a personal or agency-provided vehicle.
