Job summary

Avenues Recovery in Baton Rouge, LA is now hiring Case Managers to advocate for clients and provide comprehensive support throughout their treatment journey. You will ensure clients have access to resources and coordinate discharge planning to assist in their recovery.

Qualifications

Minimum 1 year experience in treatment industry preferred.
In recovery, a minimum one-year period of sustained sobriety is required.

Responsibilities

Advocate for client, addressing and resolving any external barriers to treatment.
Initiate case management for each client upon intake.
Oversee client accounts, ensuring insurance is active throughout duration of treatment.
Provide ongoing case management throughout the duration of treatment.
Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post-discharge.
Facilitate comprehensive discharge planning.

What You’ll Do:

Advocate for client, addressing and resolving any external barriers to treatment
Initiate case management for each client upon intake, addressing any pressing needs
Oversee client accounts, ensuring insurance is active throughout duration of treatment
Provide ongoing case management throughout the duration of treatment
Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post‑discharge
Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.)

What We’re Looking For:

High School diploma or GED preferred
Minimum 1‑year experience in treatment industry preferred
In recovery, a minimum one‑year period of sustained sobriety is required
Excellent communication, multitasking, and time management skills
Strong problem‑solving and critical thinking skills
Warm, empathetic, respectable personality
