Northeast Family Services is seeking a Case Manager to support in-home services and coordinate care with families and DCYF. The role involves documenting clinical contacts, developing measurable treatment plans, and presenting progress in court and to stakeholders.
The position requires a bachelor’s degree in a human services field, 2+ years’ experience with children and families, and a valid driver’s license. Excellent collaboration and communication skills are essential.
The Case Manager will report to the Clinical Supervisor. In collaboration with other members of the clinical team, the case manager is responsible for serving as a liaison for the treatment team, client, families and collateral service providers.
