MHA (Mental Health Association) in Massachusetts seeks a Case Manager to provide outreach, assessments, and engagement for participants in the BSAS Low-Threshold Housing Program. Focus on trauma-informed, person-centered support to help secure and maintain housing while addressing related needs.
Role includes collaboration with a multidisciplinary team, development of IAPs, and crisis management. Pay is $19-21 per hour; standard hours are Mon-Fri, 9am-5pm, with some flexibility.
The Case Manager provides outreach, assessment and engagement to participants of the BSAS Low-Threshold Housing Program using a trauma-informed, person-centered approach. Support individuals who are in substance use recovery and experiencing homelessness to transition into housing, maintain housing, and ensure their other needs are met. Provide emotional support and psychoeducation re: coping skills and harm-reduction strategies. Complete documentation thoroughly and in a timely manner. Perform responsibilities professionally, in accordance with MHA's Core Values (Respect, Integrity, and Compassion), policies, practices, program funding and applicable regulatory agency guidelines.
Pay Rate: $19-21 an hour

The Division of Supportive Housing (DSH) provides safe, stable housing and individualized wraparound services for individuals who have experienced chronic homelessness, many of whom are also navigating challenges related to substance use, incarceration, or trauma. DSH programs focus on long-term stability and help participants transition to independent, permanent housing. Through person-centered care, housing navigation, and community-based supports, we empower individuals to build a foundation for recovery, self-sufficiency, and a better quality of life.
