Job summary

Responsibilities

• Under supervision, ensure that activities of this position and relevant programs align with Catholic Charities' mission, vision, and values.
• Provide clients direct service, either in the office or in the field.
• Review and monitor services, reassess needs, and initiate services responsive to clients' needs; provide service coordination.
• Assist clients in developing formal and informal community supports.
• Provide advocacy on behalf of clients.
• Prepare and complete all required case recordings, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
• Attend clinical and administrative staff meetings, multi-disciplinary team meetings as required.
• Review treatment plans and progress with supervisor for continued support, advice, and approval.
• Perform other duties as necessary to accomplish objectives.

Under general supervision, provide case management and/or specialized supportive and supplemental services to clients to aid them in improving their social, economic, cultural or educational status or to assist them in restoring their capacity for social functioning.

• Salary ranges: $44,349.15 - $48,231.52 annually

Responsibilities

Requirements

• Combination of education and experience normally represented by a Bachelor's Degree and one to two years related professional experience.
• Must have competent oral, written and interpersonal communication skills.
• Must be well organized, detail oriented and have the ability to maintain confidentiality.
• Ability to work with diverse client population, community organizations as well as staff at all levels.
• Must be able to form good working and therapeutic relationships.
