Job summary

Catholic Charities of the Archdiocese of New York is seeking a dedicated Case Manager to provide direct eviction-prevention services. You will engage clients through advocacy, crisis intervention, information and referral, and short-term case management, working toward stable housing.

Responsibilities

  • Provide crisis intervention to individuals and families facing a housing crisis or eviction risk.
  • Conduct needs assessments and develop service plans to address identified needs.
  • Assess individuals and families and refer to Catholic Charities programs or external facilities as needed.
  • Conduct outreach and home visits.
  • Maintain communication with programs for referrals and information sharing.
  • Assist clients in applying for and receiving public and private benefits.
  • Complete documentation and statistical reports.
  • Attend trainings, supervisor meetings, and agency activities.
  • Support food pantry operations and related administrative duties.

Description

Summary : The case manager provides direct case management services, focusing on eviction prevention, to clients who are subject to or at risk of eviction. Services are provided through advocacy, crisis intervention, information and referral and short-term case management,

Salary

$45,000 - $50,000 annually

Essential Duties And Responsibilities Include The Following.
  • Provide crisis intervention to individuals and families facing a housing crisis, and who are subject to or at risk of eviction . Address emergencies related to housing, food, medical needs, spouse abuse, and any concrete need as presented by client.
  • Conduct thorough needs assessment s and develop service plan s to address identified , concrete needs using a short-term intervention model.
  • Conduct assessments of individual and family problems and follow - up with full or brief services, or with referrals to Catholic Charities programs and/or clinics or an outside facility as deemed appropriate.
  • Conduct outreach and home visits .
  • Develop and maintain good communication with other programs and departments within the organization to facilitate inter-agency referrals.
  • Assist clients in applying for and receiving public and private benefits .
  • Maintain current and accurate documentation of services provided to clients.
  • Attend regular sessions with immediate supervisor.
  • Conduct educational and/or supportive groups.
  • Attend Agency’s in-service training and unit meetings as well as any other agency-related activities.
  • Complete statistical reports.
  • Assist with food distribution at food pantry and related administrative duties.
  • Any other job-related duties assigned by the supervisor.
  • Other duties may be assigned.
Position Type and Expected Hours of Work

This is a f ull-time position. Days and hours of work are generally 9:00 AM to 5 PM Monday - Friday. Additional hours may be required to meet program deadlines, or client needs.

Working conditions and physical demands required

The demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

  • To conduct home visits and outreach within NYC at least 50% of the time , individuals will use public transportation or traverse city streets, occasionally ascend/descend stairs, and be exposed to outside weather conditions.
  • Ability to work with the public .
  • Ability to position boxes/items at different levels that weigh up to 25 lbs .
  • Remain stationary at a workstation and us e a computer at least 50 % of the time .
  • Potential to c arry a laptop to offsite work locations.
Qualifications
Education and/or experience required
  • Bachelor’s degree or at least two years ’ experience in the social services field.
Skills, Licenses, and/or competencies required
  • Bilingual, English/Spanish, a plus.
  • Excellent interpersonal skills, knowledge of social service delivery systems, and knowledge of City resources .
  • G ood organizational, time-management and communication skills.
  • Individual must possess the ability to work well independently as well as part of a team .
