Job summary

Catholic Charities, Diocese of Cleveland, is seeking a Case Manager for Lake County to provide case management and supportive services to youth and families. The role supports prevention with telehealth and in-office visits, under supervision, to improve social and economic functioning and access to community resources.

The grant-funded position runs through June 2027 and offers a salary in the range of $32,831.54 to $42,270.04 annually.

Qualifications

  • Bachelor's degree plus one to two years related professional experience.
  • Competent oral, written and interpersonal communication skills.
  • Well organized, detail oriented and able to maintain confidentiality.
  • Ability to work with diverse client population and staff at all levels.
  • Ability to form good working and therapeutic relationships.
  • Fingerprinting/background check required.

Responsibilities

  • Ensure activities align with Catholic Charities mission, vision and values.
  • Work with children, caregivers, and parents to address unmet needs and navigate barriers.
  • Provide direct service in office, via telehealth or in the field/homes.
  • Initiate and coordinate interventions, review progress, reassess needs.
  • Assist clients in developing community supports and partner with local resources.
  • Provide advocacy on behalf of clients.
  • Prepare and complete all required case recordings in EMR and reports.
  • Attend clinical and administrative staff meetings and teamwork sessions as required.
  • Review treatment plans with supervisor for ongoing support and approval.
  • Perform other duties to accomplish objectives.

Job description

Join our supportive, fun team with a flexible schedule providing in-person and virtual service delivery. Under general supervision, provide case management, specialized supportive, supplemental services to clients to aid them in improving their social, economic, cultural or educational status or to assist them in restoring their capacity for social functioning.

*Grant funded position until June 2027.

  • Pay range: $32,831.54 - $42,270.04 annually
Case Manager, Lake County

This program serves youth and it is a prevention and supplemental behavioral health program that collaborates with a multitude of community resources with the aim of providing stability and improving functioning of family members. Services also provide the caregivers opportunities to enhance their parenting strategies.

Responsibilities
  • Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
  • Work with children, caregivers, and parents to address unmet needs and support them as they navigate through their barriers.
  • Provide direct service either in the office, via telehealth or in the field/homes.
  • Initiate and coordinate interventions, review and monitor progress, and reassess needs to ensure successful outcomes.
  • Assist clients in developing formal and informal community supports, while partnering with local agencies and resources.
  • Provide advocacy on behalf of clients.
  • Prepare and complete all required case recordings in an electronic medical record, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
  • Attend clinical and administrative staff meetings, and multi-discipline team meetings as required.
  • Review treatment plans and progress of cases with supervisor for continued support, advice and approval.
  • Perform other duties as necessary to accomplish objectives.
Requirements
  • Combination of education and experience normally represented by a Bachelor's Degree and one to two years related professional experience.
  • Must have competent oral, written and interpersonal communication skills.
  • Must be well organized, detail oriented and have the ability to maintain confidentiality.
  • Ability to work with diverse client population, community organizations as well as staff at all levels.
  • Must be able to form good working and therapeutic relationships.
