Job summary

TURN is seeking a dedicated housing/case management professional in Fresno, CA. The role focuses on coordinating access to medical, educational, social, vocational, and housing resources to support program goals and whole-person care.

Responsibilities include crisis intervention, discharge planning, documentation, and housing placement facilitation, with on-site duties and potential on-call coverage. Bilingual skills and CA licensure are valued assets.

Qualifications

• Master's degree or a bachelor's degree with four years of experience in social services.
• Housing role requires two years of related experience or alternative education path as noted.
• SUD CM certification from a recognized board is preferred and registration may be required.
• Valid CA driver's license and current auto insurance required.

Responsibilities

• Provide case management with crisis intervention, discharge planning, group and individual work.
• Counsel in mental health and substance use disorders; assist clients with basic needs and symptoms.
• Complete accurate documentation and meet billable requirements if contractually required.
• Coordinate housing placements, including pre-placement visits and transportation.
• Participate in on-site housing inspections and weekly program meetings.
• Maintain confidentiality of clients' care and participate in field duties as needed.
• Support on-call shifts and other duties related to program needs.
• Ensure compliance with program requirements and organizational policies.

Job Summary

Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients.

Responsibilities

• Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
• Facilitate placement including pre-placement visits, and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
• Specialty in housing related duties.
• Facilitate placement for clients in need of housing linkages.
• Facilitate maintenance requests for clients who live on site.
• Participate in weekly housing inspections, to ensure clients are maintaining units in habitable conditions.

Qualifications

• Master's degree; or bachelor's degree with four years of experience in social services.
• For Housing two years of experience and an AA degree in social service or related field or High School Diploma and 5 years’ experience.
• For SUD CM a certification from recognized board is preferred and registration required.
• Valid CA driver’s license and current auto insurance per contract requirements required.
• Experience providing mental health and/or substance use disorder services preferred.
• Cultural competency understanding required.
• Bilingual preferred.
• Knowledge of Microsoft Office and EHR software preferred.
