Job summary

Peregrine Senior Living in the Buffalo, NY region is seeking a Case Manager to assist with resident integration and ongoing collaboration with families and healthcare teams. The role focuses on therapeutic support, DOH guideline adherence, and comprehensive case documentation.

Ideal candidates have a human services degree with related experience and a background in Skilled Nursing, Assisted Living, or Memory Care, ensuring compassionate, resident-centered care in a supportive facility

Qualifications

• Bachelor's degree in a human services field with 1 year of full-time related experience, or an Associate's degree with 3 years of full-time related experience.
• Previous case management experience required.
• Experience in Skilled Nursing, Assisted Living, and Memory Care required.

• Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to the facility, including addressing their social, emotional, and psychological needs while following DOH guidelines.
• Act as a liaison between residents, families, and other healthcare entities.
• Assist with quality assurance reviews and maintain accurate, up-to-date files and case management notes.
• Work closely with the entire care team to ensure a positive and safe environment for all residents.
• Demonstrate compassion and a strong commitment to working with the aging
