Job summary

Oneida Center for Rehabilitation and Nursing in Utica, NY is seeking a Case Manager for our Assisted Living Program. You will act as liaison between residents and the Director of Case Management, addressing concerns and coordinating social services and transportation as needed.

Ideal candidates have experience in human services, strong written and verbal communication, and the ability to multitask while maintaining meticulous documentation and prioritizing resident care. This is an on-site role.

• Work experience in human services, social services or related fields strongly preferred.
• Proficiency in MS Office & MS Excel.
• Excellent written & oral communication skills and strong problem-solving abilities.
• Superior administrative and organizational skills with the ability to prioritize.
• Multitasking and efficient time management are required.
• Exceptional interpersonal & leadership skills.

Responsibilities

• Act as liaison between residents and the Director of Case Management.
• Address resident concerns including roommate issues, room changes, and behavioral problems.
• Assist new residents in orienting to the Assisted Living Program.
• Assist residents in obtaining SSI/SSDI benefits and Access-A-Ride applications & services.
• Coordinate medical appointments, scheduling transportation and handling insurance eligibility.
• Document for readmissions from hospitalization or rehabilitation.
• Maintain incident reports and proper documentation.
• Order durable medical equipment per physician orders.
• Assist with Medicaid recertification or SSA concerns with the finance office.
• Coordinate Mental Health evaluations and coordination of services as needed.
• Act as a resident spokesperson when necessary.

Duties Include:

• Will act as a liaison between our Residents and the Director of Case Management
• Address Resident concerns, which may include;roommate issues, room changes, behavioral problems, etc.
• Help all new Residents in orienting to theAssisted LivingProgram
• Assist Residents in obtaining SSI/SSD benefits and Access-A-Ride applications & services
• Coordinate medical appointments; including scheduling transportation, insurance eligibility, etc.
• Assist with the documentation necessary for re-admissions from hospitalization or rehabilitation
• Maintain organized incident reports and proper documentation as necessary
• Order durable medical equipment based on physician orders
• Collaborate with the finance office to assist with Medicaid recertification or SSA concerns
• Ensure completion of Mental Health evaluations and coordination of services as needed
• Act as a resident spokesperson when necessary

Requirements:
