Responsibilities

  • Participate as a member of a multidisciplinary team providing holistic, values-based services.
  • Assist with the development and review of individualized recovery plans in collaboration with the program participant, clinical and medical staff, and other stakeholders identified with the participant.
  • Help determine the resources and services needed to support each participant’s individualized goals.
  • Advocate for program participants to help ensure access to appropriate services and supports.
  • Identify program needs and service gaps and recommend strategies for addressing them.
  • Develop and maintain productive relationships with community agencies.
  • Connect program participants with available community resources and services.
  • Provide community-based services to program participants.
  • Educate participants about available services, supports, and resources.
  • Assist participants with making meaningful connections in their communities.
  • Support participants in working toward the goals identified in their recovery plans.
  • Transport program participants to appointments as required.
  • Complete assessments, service documentation, and other records accurately and within required timeframes.
  • Maintain documentation in accordance with RHD requirements and applicable funder, licensing, and accreditation standards.
  • Participate in program planning, coordination, and service-review activities.

Position Summary

The Case Manager identifies the resources needed to address each program participant’s individualized needs. This position takes the necessary actions to support implementation of the participant’s service treatment plan, including advocating for access to needed services, identifying program needs and service gaps, and recommending appropriate strategies.

The Case Manager works from a holistic, team-based, and values-driven perspective while demonstrating a commitment to RHD’s values in the performance of all job responsibilities.

What You’ll Do
Team-Based Planning
  • Participate as a member of a multidisciplinary team providing holistic, values-based services.
  • Assist with the development and review of individualized recovery plans in collaboration with the program participant, clinical and medical staff, and other stakeholders identified with the participant.
  • Help determine the resources and services needed to support each participant’s individualized goals.
  • Advocate for program participants to help ensure access to appropriate services and supports.
  • Identify program needs and service gaps and recommend strategies for addressing them.
  • Develop and maintain productive relationships with community agencies.
  • Connect program participants with available community resources and services.
Direct Participant Support
  • Provide community-based services to program participants.
  • Educate participants about available services, supports, and resources.
  • Assist participants with making meaningful connections in their communities.
  • Support participants in working toward the goals identified in their recovery plans.
  • Transport program participants to appointments as required.
Documentation and Administrative Responsibilities
  • Complete assessments, service documentation, and other records accurately and within required timeframes.
  • Maintain documentation in accordance with RHD requirements and applicable funder, licensing, and accreditation standards.
  • Participate in program planning, coordination, and service-review activities.
  • Perform other job-related duties as assigned.

Working Conditions
  • This position provides services in community-based settings throughout the program’s service-delivery area.
  • Regular local travel is required.
  • The Case Manager is required to drive program participants to appointments and other approved service-related destinations.
