Avenues Recovery Center in Baton Rouge, LA is seeking a dedicated Case Manager to coordinate treatment access for clients and ensure seamless continuity of care. You will initiate intake-based case management, resolve barriers to treatment, and monitor insurance coverage throughout the program.

The role emphasizes advocacy, coordinated housing and financial support, and thorough discharge planning to support lasting recovery. Join a compassionate team committed to evidence-based care.

Qualifications

• High School diploma or GED preferred.
• Min. 1-year experience in treatment industry preferred.
• If in recovery, a minimum one-year period of sustained sobriety is required.

Responsibilities

• Advocate for client, addressing and resolving any external barriers to treatment
• Initiate case management for each client upon intake, addressing any pressing needs
• Oversee client accounts, ensuring insurance is active throughout duration of treatment
• Provide ongoing case management throughout the duration of treatment
• Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post - discharge

What You'll Do:
