Job summary

Avenues Recovery Center in Norfolk, VA seeks a case manager for its new inpatient extended care rehab facility. You will advocate for clients, initiate intake-based case management, monitor insurance activation, and coordinate housing, financial aid, legal aid, and post-discharge planning.

Qualifications include a High School diploma or GED and at least 1 year in the treatment field; sobriety is preferred if in recovery. Strong communication and organizational skills are essential for success.

Qualifications

• If in recovery, at least 1 year of sobriety is required.
• Excellent communication, multitasking, and time management skills.

Responsibilities

• Advocate for client, addressing and resolving external barriers to treatment.
• Initiate case management for each client at intake and address pressing needs.
• Oversee client accounts and ensure insurance remains active during treatment.
• Provide ongoing case management throughout treatment duration.
• Arrange housing, financial/legal aid, vocational training, and post-discharge support as needed.

Now hiring a case manager for our new inpatient extended care rehab facility opening in Norfolk, VA!

What You'll Do:

• Advocate for client, addressing and resolving any external barriers to treatment
• Initiate case management for each client upon intake, addressing any pressing needs
• Oversee client accounts, ensuring insurance is active throughout duration of treatment
• Provide ongoing case management throughout the duration of treatment
• Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post - discharge
• Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.)

What We're Looking For:

• High School diploma or GED preferred
• Min. 1-year experience in treatment industry preferred
• If in recovery, a minimum one-year period of sustained sobriety is required
• Excellent communication, multitasking, and time management skills
• Strong problem-solving and critical thinking skills
• Warm, empathetic, respectable personality
