Job summary

Lone Star Circle of Care seeks a compassionate Case Manager to coordinate care for patients, connect them with community resources, and support health and wellness goals. This clinic-based role requires regular in-person attendance and travel to LSCC facilities as needed.

Responsibilities include conducting biopsychosocial assessments, facilitating referrals, tracking referrals, and collaborating with care teams to integrate primary and behavioral health services.

Qualifications

  • Bachelor's degree in Social Work from an accredited college or university.
  • Minimum two years of experience as a case manager with a CCM (Certification in Case Management).
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.

Responsibilities

  • Assist clients and families in identifying and accessing community resources to alleviate health care needs.
  • Deliver case management, care coordination, and crisis intervention services.
  • Manage and track all aspects of the patient referral process, ensuring compliance with policies and timelines.
  • Educate families on the implications of their medical condition and its impact on lifestyle.
  • Serve as a liaison and coordinates care between providers, treatment providers, community groups, and social service agencies.
  • Maintain accurate and up-to-date referral information and initiate referrals as appropriate.

Job description

Lone Star Circle of Care is seeking a compassionate and dedicated Case Manager to join our team. In this role, you will work closely with patients to coordinate care, connect them with community resources, and support them in achieving their health and wellness goals. Our Case Manager serves as liaison and coordinates care for patients with biopsychosocial needs in the context of medical, emotional and/or behavioral problems. Assess for such needs at the individual level, as well as facilitating and tracking successful referral to resources. Participates in population-based Care Management supporting the integration of primary and mental health care to treat the whole patient.

Candidates with experience working with individuals affected by Substance Use Disorders (SUD), addiction recovery, behavioral health, or related community-based services are strongly encouraged to apply. Experience navigating complex social, behavioral, and healthcare needs is highly valued.

This position is primarily clinic-based and requires regular in-person attendance to provide direct patient support, collaborate with care teams, and build meaningful relationships with patients. Applicants should be comfortable working on-site and engaging face-to-face with patients, providers, and community partners. This position requires travel to various LSCC facilities as needed.

A Day in the Life of a Case Manager may look like this:
  • Assist clients and/or families in identifying and accessing community resources to alleviate social, environmental, and/or economic problems impacting health care needs.
  • Deliver case management, care coordination, and crisis intervention services to individuals and families, within the scope of practice.
  • Manage and track all aspects of the patient referral process, ensuring compliance with organizational policies and timelines.
  • Educate families on the implications of their medical condition and its impact on lifestyle.
  • Serve as a liaison and/or coordinates care between the client's providers, other treatment providers, community groups, and social service agencies.
  • Maintain accurate and up-to-date referral information, and initiate referrals as appropriate.
  • Refer clients and/or their families to community resources (programs, agencies, other providers, etc.) to assist in alleviating social, environmental, and economic problems affecting health care needs.
  • Update and maintain resources and contact points for providers as needed.
  • Adhere to patient care standards in alignment with LSCC health education and information guidelines.
  • Demonstrate a thorough understanding of national patient safety initiatives by consistently following all LSCC safety protocols and procedures.
  • Participate in data collection, focus groups, TJC, PCMH, and other quality improvement initiatives.
  • Maintain accountability for ongoing professional development and for sharing knowledge with others.
  • Responsible for knowledge of and compliance with all LSCC policies and procedures.
We ask our Case Manager to possess a minimum of:
  • Bachelor's degree in Social Work from an accredited college or university OR
  • Minimum two (2) years of experience as a case manager with a CCM (Certification in Case Management) in a community-based and/or medical setting with an understanding of behavioral health prevention science.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross
The following experience/skills are preferred:
  • Master's degree in Social Work
  • Experience in substance abuse screening, use, and/or treatment
  • Experience with screening, brief intervention, and referral to treatment (SBIRT)
  • Experience with motivational interviewing (MI) and smoking cessation services
  • Experience working in behavioral health and/or human services
  • Proficiency with Electronic Medical Record (EMR) system, as well as computer and web-based interfaces
  • Bilingual English/Spanish language skills
