The Senior Coordinator provides administrative and operational support to Case Managers and Social Workers in a fast-paced acute care hospital environment. This role assists with patient discharge planning, coordination of post-acute services, documentation management, insurance and referral processes, and communication between patients, families, physicians, and community agencies. The Senior Coordinator prioritizes multiple tasks simultaneously, ensures timely follow-up on discharge needs, and supports efficient patient throughput while maintaining excellent customer service and attention to detail. Strong organizational skills, adaptability, and the ability to thrive in a demanding healthcare setting are essential.
The Senior Coordinator for Case Management would work under the direction of the RN Care Managers and Social Workers. This position functions with his/her peers and other care providers for problem solving and facilitating in-patient and post hospitalization care. And coordinate, oversee records and transmit information pertinent to the resource management of patients.
Associate's degree or High School Diploma and equivalent relevant experience required. Bachelor's degree preferred. Medical assistant or Licensed Practical Nurse (LPN) highly preferred. 5-7 years of customer service, Insurance knowledge medical assistance or secretarial experience preferred. Prior experience in a medical setting required Ability to organize and utilize work hours effectively and with minimal supervision Medical terminology is a must Must be able to use Outlook, EPIC, Teams, Faxing, and phone knowledge.
