Job summary

Trinity Health is seeking a Senior Coordinator, Case Management & Social Services to provide administrative and operational support to Case Managers and Social Workers in a fast-paced hospital setting. The role focuses on discharge planning, post-acute coordination, documentation management, and payer communications to ensure smooth patient throughput.

Under RN Care Managers and Social Workers, you will coordinate records, transmissions, and referrals for post-hospital care, while maintaining

Qualifications

• Associate's degree or high school diploma with equivalent experience is required.
• Bachelor's degree preferred.
• Medical assistant or LPN is highly preferred.
• 5-7 years of customer service, insurance knowledge, medical assistance or secretarial experience preferred.
• Prior experience in a medical setting is required.
• Must be able to use Outlook, EPIC, Teams, Faxing, and phone knowledge.

Responsibilities

• Communicate information received from payers to utilization review nurse.
• Transmit continued stay reviews and track authorizations.
• Complete scheduling and follow-up appointments.
• Fax and call agencies and facilities to assist with discharge referrals and continuity of care.
• Assist with delivery of charity items and arrange transportation as needed for discharge.
• Complete PCS forms and compile packets for transport.
• Coordinate relationships with community partners.
