Advanced Medical Management, Inc. seeks a Case Management Manager to report to the Senior VP of Clinical Operations and oversee clinical and operational aspects of case management.

The role drives client stability, wellness, and autonomy through advocacy, assessment, planning, communication, and care coordination. The manager will guide both inpatient and outpatient teams, develop staff education, and participate in platform development while maintaining HIPAA compliance and high-quality care

Formación

• Bachelor’s degree required; Master’s Degree preferred in nursing or health-related field.
• Minimum of 5 years of UM and CM experience in an IPA/PMG or HMO setting.
• Experience in management (hiring, counseling, terminations).
• Maintains confidentiality and adheres to HIPAA policies.
• Managerial skills to manage on-site and remote team.
• Strong clinical judgment to interpret medical guidelines.
• Analytical skills to review metrics.

Responsabilidades

• Review inpatient cases and guide IP case managers on reviews, transfers, observation, discharge planning, and transitions of care.
• Review outpatient cases and guide OP case managers on reviews and disease management processes.
• Inspire the team with creative ways to improve care and lead Monday nurse team meetings.
• Identify and develop educational needs; motivate team to set and reach improvement goals.
• Enhance systems case management usage and participate in platform development projects.
