Baystate Health is seeking a Supervisor, Case Management to provide leadership and oversight of case management and utilization review activities. This role supports effective patient care coordination, appropriate resource utilization, and timely transitions across the healthcare continuum.

Working with clinical teams, physicians, and hospital leadership, the supervisor promotes quality patient outcomes, efficient operations, and a positive team environment.

Qualifications

• Associate Degree in Nursing (ADN) is required.
• Current Massachusetts RN license.
• Minimum of three years of case management experience.

Responsibilities

• Oversee daily case management and utilization review workflows, assignments, and staffing.
• Lead team orientation, training, scheduling, performance management, and professional development.
• Provide guidance to case managers on evidence-based practices and clinical guidelines.
• Participate in case management and utilization review committees and liaise with the Physician Advisor.
• Collaborate with Denials and Appeals Specialists on utilization management concerns.
• Assess admission appropriateness, level of care, and continued stay using screening criteria.
• Communicate findings and level-of-care concerns to the healthcare team and Physician Advisor as needed.
• Monitor patient progression, length of stay, and discharge needs for timely transitions.
• Address barriers to discharge with physicians and interdisciplinary teams.
• Educate physicians and team members on length of stay and medical necessity.
