Job summary

Requires an RN/LPN license or a Bachelor's degree in Social Work or related field; 3 years of healthcare experience preferred and case management certification preferred.

Qualifications

• Graduate from an accredited nursing program with current state RN or LPN license.
• Bachelor’s degree in Social Work, Psychology, Sociology, or related field.
• Three years of nurse or social work experience in health care preferred.
• Certification in Case Management preferred.
• Knowledge of case management methods, practice, and procedure.
• Knowledge of Medicare/Medicaid reimbursement and community resources.

Responsibilities

• Plan, coordinate, monitor, and supervise services in Home and Community Based Services.
• Provide direct supervision to the Case Management Team and Administrative Assistant staff.
• Demonstrate ability to work under pressure, meet deadlines, and handle changes.
• Follow licensing, agency requirements, Benefis policies, and regulatory procedures.
