Job summary

Community Housing Innovations, Inc. in Midtown Manhattan seeks a Case Management Supervisor to lead a team of Case Managers, ensuring tailored service and care plans for residents that foster stabilization and independence. The role coordinates external partnerships and provides direct supervision to staff.

The position requires a relevant bachelor’s degree, several years of supervisory experience, strong English skills, and the ability to manage care plans and documentation in CARES and AWARDS.

Qualifications

• Bachelor's degree in Social Work or Psychology; five years' social services experience plus two years' supervisory experience.
• Seven years total case management experience and three years supervisory experience in the past ten years may substitute for education.
• Proficiency in English; multilingual and Spanish proficiency prioritized.
• MS Word, Excel, Outlook; CARES, HOME, CAPS and AWARDS prioritized.

Responsibilities

• Ensure residents receive quality services per organizational principles and program requirements.
• Review and customize care plans to promote independence and stability.
• Develop partnerships with external providers for immigration, health, benefits, and employment services.
• Supervise Case Managers; conduct performance reviews and provide coaching.
• Document interactions in CARES and AWARDS; ensure compliance.
• Coordinate discharge planning and public benefits enrollment.
