Qualifications

• An earned master's degree from an accredited institution.
• Previous supervisory and mental health experience.
• Valid driver’s license in good standing.

Responsibilities

• Perform outreach and engagement to determine consumers’ eligibility.
• Provide hospital liaison support to Supervisor.
• Assist residents in understanding their rights and responsibilities under a tenant lease arrangement.
• Collaborate with consumers, family members, and other providers to develop Individualized Rehabilitative Plans.
