Job summary

Oaks Integrated Care is actively seeking for a qualified Case Manager for our ARRIVE program in Lumberton, NJ. The role requires an MSW or related degree with clinical training, or a bachelor’s degree with 1 year post-degree experience, plus a valid driver’s license.

Duties include intensive community-based services, care coordination, advocacy, and integration with multiple providers. Schedule: Mon-Fri with some evenings/weekends as needed; team-oriented environment and opportunities for

Qualifications

• Must have an MSW or Master’s degree with clinical training, OR a related bachelor’s degree with 1 year post-degree experience.
• Valid driver’s license in good standing is required.

Responsibilities

• Provide intensive community-based services to seriously and persistently mentally ill individuals in the community.
• Facilitate service linkage within mental health and other health systems across multiple providers.
• Provide direct service support including transportation, coordinating treatment teams, applications for services, advocacy, escorts to appointments, daily living skills, and money management.
