Job summary

Opportunity Works, Inc. seeks a case management professional to implement and manage the Case Management System. You will collaborate with the Interdisciplinary Team to help individuals served reach their goals and ensure documentation meets policy, licensing, and accreditation standards.

Responsibilities include advocacy and support for individuals in ABI, CBDS and Day Habilitation, requiring strong organizational and communication skills and a valid driver's license.

Qualifications

• Bachelor's degree preferred, minimum high school diploma or equivalent.
• Experience with case management and case reviews in a health care setting.
• Excellent organizational and verbal and written communication skills.
• Valid driver's license and ability to drive a van preferred.
• Crisis intervention training (e.g., PABC) is favorable.
• CPR, First Aid and MAP certificates preferred.

Responsibilities

• Responsible for the implementation of the Case Management System.
• Work with the Interdisciplinary Team to help individuals reach goals.
• Ensure documentation is completed and filed per policy and standards.
• Provide advocacy and support to individuals in ABI, CBDS and Day Habilitation.
