Job summary

OPPORTUNITY WORKS INC. is seeking a Case Manager to implement the Case Management System and work with an Interdisciplinary Team to help individuals served reach their goals.

The role emphasizes documentation accuracy and adherence to licensing standards, with advocacy and support across ABI, CBDS and Day Habilitation programs.

Qualifications

• Bachelor’s degree preferred; minimum HS diploma.
• Experience with case management and case reviews.
• Excellent organizational and written/verbal communication skills.
• Valid driver’s license; ability to drive a van preferred.
• Crisis intervention training (PABC) favorable; CPR/First Aid/MAP preferred.

Job description

Responsible for the implementation of the Case Management System. The Case Managers work with the Interdisciplinary Team to ensure that all people supported are reaching their goals. Ensure that all documentation is completed and filed according to Opportunity Works Policy and all Licensing and Accreditation Standards. Provide advocacy and support to individuals served in the ABI, CBDS and/or Day Habilitation.Qualifications:Bachelor’s Degree preferred, minimum High School Diploma or equivalentExperience with Case Management and Case Reviews in a relevant health care settingThis position requires excellent organizational skills as well as excellent verbal and written communication skills.Valid driver’s license and the ability to drive a van preferredCrisis intervention training (i.e. PABC) is favorableCPR, First Aid and MAP certificates preferred
