Penn State Health St. Joseph Medical Center in Reading, PA is seeking a Manager of Case Management to lead care management and social work with 24-hour accountability.

You will oversee clinical operations within annual budgets and ensure regulatory compliance, supporting the organization’s mission and values. Qualified candidates will have PA nursing licensure, 5–7 years in clinical or case management across areas, 3–5 years in leadership, and proven mentoring abilities.

Qualifications

• Current PA nursing license.
• 5-7 years of clinical or case management experience across multiple clinical areas.
• 3-5 years of leadership experience.
• Strong management leadership skills.
• 3-5 years of experience with case management.
• Strong mentoring and coaching experiences.
