Job summary

Penn State Health’s St. Joseph Medical Center in Reading, PA, seeks a dedicated RN Care Coordinator in Case Management to coordinate care across the continuum and collaborate with patients, families, and the health care team.

The successful candidate will assess, plan, implement, monitor, and measure interventions for patient treatment and transitions, applying InterQual/Milliman criteria when appropriate and ensuring age-specific care protocols.

Qualifications

• Two years of nursing experience in an acute care hospital.
• Current RN license valid in the state of employment or holds a multistate RN license.
• AHA BLS required prior to end of orientation period.

Responsibilities

• Coordinate care and services for selected patient populations across the continuum.
• Collaborate with patient, family, physician and health care team to achieve optimal outcomes.
• Assess, plan, implement, monitor and measure interventions for treatment and transitional needs.
• Apply age-appropriate care protocols and promote teamwork through effective collaboration.

Skills

Education
