Job summary

Penn State Health St. Joseph Medical Center in Reading, PA is seeking a Manager of Case Management to lead care management and social work with 24-hour accountability.

You will oversee clinical operations within annual budgets and ensure regulatory compliance, supporting the organization’s mission and values. Qualified candidates will have PA nursing licensure, 5–7 years in clinical or case management across areas, 3–5 years in leadership, and proven mentoring abilities.

SUMMARY OF POSITION

The Manager of Case Management is a member of the Management Team. He/she has twenty-four hour accountability for the care management and social work, managing within the parameters of the annual personnel and supply budget. The Manager assures compliance with all requirements of regulatory bodies. This individual is held accountable within the context of philosophy and mission of Saint Joseph Regional Health Network.

MINIMUM QUALIFICATIONS

• Current PA nursing license

• 5-7 years of clinical or case management experience across multiple clinical areas

• 3-5 years of leadership experience

• Strong management leadership skills

• 3-5 years of experience with case management

• Strong mentoring and coaching experiences

PREFERRED QUALIFICATIONS

• Master's degree
