Penn State Health Hampden Medical Center is seeking a Manager of Care Coordination to lead the care management and social work teams. This role has 24-hour accountability for operations, budget adherence, and regulatory compliance across the care management function.
The manager will drive quality, staffing, and process improvements in partnership with hospital leadership, supporting both Penn State Health Holy Spirit and Hampden Medical Centers.
