Ascension Saint Thomas West in Nashville, TN is seeking a Nurse Leader for Case Management. You will supervise case managers and professional staff, coordinating disease management programs and overseeing transitions of care to optimize patient outcomes.

Team Leadership: Manage assigned case managers, professional associates, and clerical staff to ensure productive workflows.
Operational Oversight: Coordinate disease management programs and track patient outcomes throughout the post-acute period.
Resource Management: Contract with internal and external healthcare resources to secure seamless transitions of care.
Interdisciplinary Collaboration: Communicate regularly with administration and medical staff regarding admission appropriateness, length of stay (LOS), and discharge barriers.
Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.
Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.
High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
Master's degree preferred.
3 years of experience preferred.
1 year of leadership or management experience required.
