Ascension Saint Thomas West in Nashville, TN is seeking an experienced Case Management leader to oversee a team of case managers and a wide range of staff within the Case Management department. This role is Full-Time with daytime hours.

You will coordinate disease management programs, track patient outcomes during the post-acute period, and secure seamless transitions of care by partnering with internal and external healthcare resources.

Qualifications

• Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.
• Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.
• High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
• Master's degree preferred.

• Coordinate disease management programs and monitor patient outcomes post-acute.
• Contract with internal and external healthcare resources to ensure seamless transitions of care.
• Communicate with administration and medical staff about admission appropriateness, LOS, and discharge barriers.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.

Education

• High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
• Master's degree preferred.

Work Experience

• 3 years of experience preferred.
• 1 year of leadership or management experience required.
