Qualifications

• Strong oral and written communication skills.
• Knowledge of case management principles, procedures, and practices.
• Ability to adapt to a variety of duties and shifting priorities.
• Ability to negotiate and resolve conflicts between parties.
• Valid driver's license and proof of automobile insurance.

Responsibilities

• Lead, mentor, and develop a team of case managers, creating a positive and high-performing work environment.
• Recruit, train, and retain top talent to support service demands.
• Drive program development and continuous improvement to enhance service quality.
• Monitor performance, quality assurance standards, and regulatory compliance.
• Partner with counties and stakeholders to ensure contracted services meet guidelines.
• Oversee assessment, planning, referral, coordination, monitoring, consultation, and advocacy.
• Collaborate on strategic planning, budgeting, and service enhancements.
• Build professional relationships with county partners and agencies.
• Promote case management services through committees, conferences, and organizations.
• Develop and monitor policies and procedures with leadership.
