Job summary

PRC Baker Places is seeking a Case Management Program Director at our San Francisco headquarters. You will lead agency-supported cooperative housing programs and case management services, aligning with mission and strategic priorities to improve client care and outcomes.

You will supervise staff, monitor performance, ensure Medi-Cal compliance, and report program data. The role requires collaboration with senior leadership and community partners and may include on-call duties.

Qualifications

• Active licensure in California with one of the listed licenses.
• Minimum three years of behavioral health program management experience.
• Experience supervising and developing staff in a community-based setting.
• Knowledge of Medi-Cal documentation and county contracts.
• Proven ability to monitor performance and drive quality improvement.

Responsibilities

• Lead agency-wide cooperative housing programs and case management services.
• Develop and monitor policies to improve client care and outcomes.
• Ensure compliance with clinical standards and regulatory requirements.
• Recruit, supervise, train, and evaluate program staff.
• Monitor staff productivity and ensure service units are met across categories.
• Maintain timely clinical documentation per standards.
• Analyze client data to measure outcomes and support QA initiatives.
• Represent the agency in meetings and community partnerships.
• Support on-call duties and other assigned tasks.
