Job summary

PRC Baker Places in San Francisco seeks a Clinical Case Manager to provide case management, therapy, crisis response, and psychosocial assessments for adults with MH/SUD and housing-related needs.

The role collaborates with the Case Management Program Manager and other teams. Active BBS licensure (LCSW/LMFT/Psy.D./ASW/AMFT/LPCC/APCC) and 2 years of relevant experience are required. CPR/First Aid certification is expected.

Qualifications

• Active California licensure with BBS (LCSW/LMFT/Psy.D./ASW/AMFT/LPCC/APCC).
• Minimum 2 years clinical experience with adults with MH/SUD and/or homelessness.
• Knowledge of San Francisco community resources and harm-reduction practices.

Responsibilities

• Provide individual behavioral health counseling and case management for adults with MH/SUD needs.
• Provide group therapy and rehabilitation for MH/SUD clients.
• Coordinate with providers to access primary medical care and mental health services.
• Outreach to AILP clients at their residence for on-site services.
• Conduct initial and updated psychosocial assessments.
• Develop and adjust individualized treatment plans.
• Documentation to meet agency standards and timeliness.
• Provide culturally competent services using harm reduction and trauma-informed approaches.
• Respond to mental health crises and provide immediate access to services and follow-up.
• Rotating on-call duties outside business hours as needed.
• Maintain a productive caseload per contractual requirements.
• If licensed, complete and maintain 51/50 certification.
• Provide step-down case management for housing transitions.
• Promote agency mission and maintain licensure and training.
• Maintain CPR and First Aid certifications.
• Comply with annual TB testing requirements.
• Additional duties as assigned.
