Valley Health in Winchester, VA is seeking a Case Management Program Assistant to support the Case Management team with documentation, admissions, updates and discharge information. You will assist with charting and coordinate patient care under supervision of a nurse or social worker.

Strong clerical skills, the ability to prioritize tasks, and good judgment are required. A high school diploma or GED is required, with 1 year clerical experience and basic medical terminology preferred.

Responsibilities

• Assist CM team with documentation of admissions, updates and discharge information.
• Withdraws/inserts information into patient charts and facilitates care delivery.
