Encompass Health in Petersburg, VA is seeking a Case Management Assistant to streamline workflow, coordinate referrals, admissions, and schedules, and support daily Case Management operations across our patient-care teams.

This role emphasizes collaboration with clinicians, families, and community resources, with flexibility for various shifts and a path for growth within our nationwide inpatient rehabilitation network.

Qualifications

• High school diploma or equivalent preferred.
• Proficiency in Microsoft Office products.
• Strong typing skills with speed and accuracy.
• Knowledge of medical terminology (preferred).
• Experience with insurance/payor systems (preferred).
• Good visual acuity and communication skills.
• Flexibility to work weekdays/weekends, evenings, or night shifts if necessary.
• Availability during religious/legal holidays as scheduled.

Responsibilities

• Coordinate and facilitate Team Conference meetings.
• Gather, take action on, and maintain Case Management documentation such as referrals, admissions, orders, and schedules.
• Effectively coordinate communication of daily Case Management Assignment updates.
• Complete pre- and post-discharge activities to ensure patients, families, and clinicians are aware and prepared.
• Gather Quality Improvement and department Performance Improvement data.
• Assist with collection of Key Care Indicators and clinical outcomes data.
• Assist in regulatory and joint commission survey preparedness activities.
