Job summary

Northern Cochise Community Hospital is seeking a dependable Case Management Assistant / Nursing Admin Assistant to support the RN Case Manager and Nursing Department in discharge planning, insurance authorizations, and care coordination.

The ideal candidate will be detail-oriented, with strong communication and organizational skills, comfortable handling confidential information, and proficient with EMR documentation to ensure seamless patient transitions.

Qualifications

• Minimum 6 months of hospital experience; 1 year of hospital experience preferred.
• Current BLS/CPR certification required in accordance with Northern Cochise Community Hospital policy.
• Current certification in non-violent crisis intervention (CPI) required.
• Ability to maintain confidentiality of medical, financial, employee, computer, and other sensitive information.
• Strong written and verbal communication skills.

Responsibilities

• Assist the Case Manager and Nursing Department with Administration duties and discharge planning and coordination of patient care needs.
• Prepare and review packets, obtain insurance authorizations, and support utilization review requests by phone, fax, and website updates.
• Coordinate durable medical equipment, outpatient services, transportation, transfers to another level of care, and follow-up appointments as directed.
• Provide patients with access to available primary care providers and community resources prior to discharge.
• Document appropriate case management information in the Electronic Medical Record.
• Attend weekly interdisciplinary team meetings and assist with accurate documentation.
• Answer phones, route calls, take messages, and provide routine information as needed.
• Develop and maintain records and files for efficient departmental operations.
• Operate standard office equipment, including computer, fax, copier, calculator, and related tools.
• Perform related duties as assigned.
