Job summary

Del Sol Medical Center in El Paso is seeking a Case Management Assistant to support RN Case Managers and Social Workers in facilitating patient discharges and coordinating post-acute placements. You will handle administrative tasks, referrals, communications, and tracking to ensure timely discharge planning.

The role requires healthcare exposure and an associate degree is preferred. CNA or EMT credentials are a plus; no travel required.

Qualifications

• Medical knowledge and terminology.
• Minimum of one year experience in a health care setting.
• Associate degree preferred.
• CNA or EMT certification preferred.

Responsibilities

• Coordinate with RN Case Manager and Social Worker for support needed.
• Communicate updates on progress throughout the day and refer patient questions back to the Case Manager/Social Worker.
• Provide clerical support in multidisciplinary rounds, including updating NATE Tempo at direction of team.
• Make referrals through web-based referral system to post-acute care providers.
• Monitor patients with PAC referrals and intervene to expedite discharge.
• Prioritize workload to ensure timely contact and intervention with PAC providers.
• Communicate with RN Case Managers/Social Workers regarding PAC referrals status, bed availability, and other barriers.
