Job summary

UNM Hospital is seeking a full-time Case Manager in the OP Care Management Svcs department to support admissions, discharges, and referrals. You will coordinate services for diverse patient populations across neonate to geriatric care under supervision of the department director.

Responsibilities include arranging patient logistics, coordinating home health and equipment, communicating plans to the care team, and ensuring compliance with hospital policies and patient safety guidelines.

Qualifications

• High School or GED required.
• Two years of related experience preferred.
• Bilingual abilities in English and another language are advantageous.

Responsibilities

• Arrange meals, lodging and transportation for families.
• Coordinate home health services and equipment as needed.
• Communicate discharge plans and referrals to the treatment team.
• Assist in patient care planning and follow-up actions.
