Job summary

BayCare Health System is seeking a Case Management Assistant at Winter Haven Hospital in Florida. This PRN as-needed role supports patient access to community services, coordinates regulatory documents, and educates patients on discharge appeal processes across hospital and community settings.

The position emphasizes collaboration with hospital staff, patients, families, and payor resources to ensure smooth transitions and effective care coordination.

Qualifications

• Preferred Associate's

Responsibilities

• Assist with tasks to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
• Distribute regulatory documents to identified patients
• Educate patients about the appeal process related to their discharge
• Support seamless transition of patients from inpatient to community services using hospital processes
• Frequent contact with patients, family, hospital staff, community resource agencies and payor representatives

• Status: PRN As needed
• Schedule: 8:00 AM - 4:30 PM
• Weekend Requirement: Occasional
• On Call: No

The Case Management Assistant Responsibilities Include

• Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
• Responsible for assisting with distributing regulatory documents to identified patients
• Educating patients about the appeal process related to their discharge
• Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
• Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives
